It is simple to register as a carer with your GP. Some surgeries have a form on their website

What are SNOMED codes?

SNOMED codes are used on electronic records by healthcare practitioners as way to identify and support patients. There are SNOMED codes for a patient who themselves provide care, a child providing informal care and carer has a carer contingency/emergency plan.

These codes are:

224484003 – Patient themselves providing care

204091000000106 - child providing informal care

1366321000000106 – Has Carer Contingency/Emergency Plan
