Supporting you to bring a family member or friend home from a hospital stay

Our new campaign aims to help you find clarity when your friend or a family member is being discharged from the hospital. We want you to know we are here for you to help to de-mystify the process, so we have created a new guide to advise on preparing for discharge from hospital.

Looking after someone who cannot cope without you can be the most rewarding thing you can do, but we also know that sometimes unexpected things may happen. When your friend or family member is in hospital and needs support, we know it can be stressful. Let us help you. We want you to feel confident in preparing your home when they return and understanding their needs.

We can help you

We have created a handy pocket guide to help you prepare for your friend or family member to return home. This will give you advice on the discharge process to help bring clarity and provide the tools you need to feel confident during this time. It includes a flow chart to help you through the discharge process and tips and prompts for making the transition back home as smooth as possible. The guide also includes some helpful links to signpost you to support and general advice.
